QUESTION: my flower beds were all complete with plants and mulch, now they are rising out of the ground.  The mulch and weed paper is now way above the flower bed bottom.  It looks horrible.  Any suggestions on what might be causing it?

My only guess is that the plants weren't planted deep enough or firmed down enough from the outset. Try adding more mulch and keep the plants well watered, if they are rising from the ground

QUESTION: It looks like there might be some mole holes.  they are deep and are uprooting the dirt.  Do you think we have an animal living in our flower beds? Thanks for your help.

that would really explain everything and moles are quite common. Just go to your local hardware store and they'll give you something, but it's not easy to get rid of them.
Please try to find an ecological way of removing them, like ultrasound.
