All I remembered was perennial rye and fescue
but not the soil type and what to be sure it was mixed with

Not to worry - I lose papers out of my car ALL the time. In fact, my husband thinks that anything that's missing must be in my car somewhere, because he calls it the "Compost-mobile" but that is another story.
I told you that the best thing to use is a combination of loam and compost - most garden centers will have this. Use as deep a layer as you can afford - four inches or more! And you remember right about the perennial rye and fescue mix - you may find mixes with both tall and fine fescues and that is fine.
