Got this plant a few years back, pitchers looked like ventrata. Managed to split into 3 plants but now the pitchers looked kind of funny. What could be happening?

I just picked up your question out of the question pool.
This sort of lidless appearance of pitchers is common when a plant is pitchering for the first time again after not pitchering for awhile. They seem to do it sometimes when conditions are a bit dry also, but usually start making more normal looking ones later as the plant grows. You mentioned recent transplanting, so this is very common.
I also noticed quite a bit of mineral deposit on the bottom of the pot. Although Nepenthes are quite tolerant of hard water, you might be getting a bit of a build-up. This could also contribute to the misshapen pitchers some. Try watering with rainwater or distilled water for awhile.
